Information provided via this form is not accepted through any other channel or by general website visitors.

The Customer Setup Form gathers, but is not limited to, the following details:
- General Business Information: legal name of your business (required); whether you have a DBA; whether your business is taxable or tax-exempt; what type of entity your business is (proprietorship, partnership, LLC, corporation); where to send a current certificate of exemption if your business is tax-exempt (billing@roofingsource.com); how many buildings you own, operate or manage; and your FEIN.
- Contact Information: primary accounts payable contact details including full name, title, email and phone (all required), and business partner or co-owner information if applicable.
- Billing Information: primary billing address (required); whether purchase orders are required on your invoice; payment preference details, including credit card information (cardholder’s name, card number, expiration date, CVV2) if credit card payment is selected; whether the billing address for the credit card is the same as the primary billing address; and the option to enroll in automatic renewals for the RoofCare program.
- Consent and Verification: agreement to terms and conditions, including consent for Eagle Trading International Corp. (dba RoofingSource) to obtain a consumer/business credit report as necessary, consistent with the Federal Fair Credit Reporting Act; certification that the applicant is an authorized signer or authorized to act on behalf of the company; and fields for the applicant’s name, signature, date, and referral source.
